In preparation for the upcoming IDHHC Commission Meeting, the online survey will be closed on Friday March 15, 2024 at 12pm.
Please take a few minutes of your time to complete the survey. Your participation is 100% voluntary and the responses will be kept completely anonymous.
The survey includes ASL video with captions, transcript for the DeafBlind, and Spanish version (Versión en Español).
Your responses to the survey can be submitted online, in an ASL video, or by appointment with IDHHC staff via telephone or videophone.
